11/14/86 13" by 15 3/4" award given to Julius Erving on his last visit to Phoenix. Plaque has color photo of Erving receiving the award with the words "Julius Erving The Doc's Last House Call Phoenix Suns vs Philadelphia 76ERS Arizona Veterans Memorial Coliseum November 14th 1987 Phoenix Arizona". Signed at the top in bold perfect looking 10/10 black felt tip pen by Julius Erving.
